Description:

Candidates must reside: within Marion county IL or surrounding counties
Training will be conducted remotely via WebEx
Candidate will travel approximately 75% of the time within the region seeing members at home or in supportive living facilities

Schedule- Monday through Friday 8AM - 5PM

Experience Required:

**Must have Case Management experience*
Must have degree in human services field, Bachelor's or non-licensed Master's level*

*Case management and discharge planning experience preferred*
Managed Care experience preferred*
Microsoft Office experience preferred

Duties-

Coordinates case management activities for Medicaid Long Term Care/Comprehensive Program enrollees.

Conducts comprehensive evaluation of Members using care management tools and information/data review

Coordinates and implements assigned care plan activities and monitors care plan progress

Utilizes influencing/ motivational interviewing skills to ensure maximum member engagement and promote lifestyle/behavior changes to achieve optimum level of health

Provides coaching, information and support to empower the member to make ongoing independent medical and/or healthy lifestyle choices

Utilizes case management and quality management processes in compliance with regulatory and accreditation guidelines and company policies and procedures.
